The “We Call & Care Program” is available to residents of the City of Northwest on a volunteer basis to those citizens that want the service. The Northwest Police Department will call the individual signed up for the program and check on them daily at a pre-set determined time. If a Northwest Police Department Officer is unable to make contact with the resident, the Officer will respond to the residence and make sure the citizen is okay. This form will provide the necessary information to the Northwest Police Department Officers in the case of an emergency, or the Officer is unable to make contact with the subscriber. The information on this form is CONFIDENTIAL and will only be used in case of emergencies or when officers respond to the residence.
